Dark circles are often blamed on lack of sleep. But pigmentation, visible blood vessels, genetics, or facial anatomy may also play a role. Dermatologist Dr. Susanne Steinkraus explains why the cause matters when choosing the right treatment – and when caution is advised.

Caffeine serums, cooling eye patches, specialized eye creams, hyaluronic acid — anyone hoping to get rid of dark circles will find no shortage of supposed solutions. But not every treatment suits every type of dark circle, and some can actually irritate the delicate eye area or make things look worse. In this interview, dermatologist Dr. Susanne Steinkraus explains which home remedies and active ingredients genuinely make a difference, when aesthetic treatments may be worth considering, and why hyaluronic acid in particular calls for especially careful thought.
How and why do dark circles develop? Are there different types?
Dark circles are not a single, uniform phenomenon. What we perceive as dark shadows can stem from pigmentation, visible blood vessels, individual anatomy, or puffiness — and often several of these factors are at play at the same time.
Dermatologically, we distinguish four main types. Pigmentation-related dark circles appear brownish and are caused by an increased deposit of melanin — due, for example, to genetic predisposition, UV radiation, hormonal metabolic disorders, or chronic irritation.
Vascular dark circles appear bluish, violet, or reddish. Because the skin on the lower eyelid is exceptionally thin and has very little subcutaneous fat, blood vessels and the underlying muscle can show through visibly.
Structurally caused dark circles are the result of anatomy: a pronounced tear trough, volume loss, protruding fat pads, or a descent of the mid-face can all create shadows. Swelling-related dark circles, on the other hand, are caused by fluid retention. They tend to be most pronounced in the morning and can be worsened by allergies, sleeping position, alcohol, a high-salt diet, lymphatic drainage problems, or genetically determined under-eye bags.
Before starting any treatment, it is important to first identify which factor is actually dominant. A cream can improve skin quality, for instance, but it cannot correct an anatomically pronounced tear trough.
Most people think of sleep deprivation when they hear "dark circles." Is too little sleep actually the most common cause?
Sleep deprivation is a relevant contributing factor, but far less often the sole cause than is commonly assumed. There are no reliable figures on how many people are affected exclusively by insufficient sleep.
After a short night, the skin can look paler and less well-perfused, making blood vessels more visible. At the same time, fluid retention may increase. Rubbing tired eyes also causes irritation and, over time, can potentially worsen pigmentation.
Anyone who has persistent dark circles despite getting enough sleep should not expect to solve the problem simply by sleeping more. In most cases, the underlying cause is genetic predisposition, pigmentation, visible vessels, or anatomical structure. Lack of sleep then merely makes existing dark circles more noticeable.
Why are dark circles often especially visible in the morning?
When lying down, fluid is distributed differently throughout the tissue and can accumulate overnight in the delicate under-eye area. Since the skin there is very thin and the lymphatic system drains fluid only with a delay, the eyelids often appear puffier in the morning. The shadows this creates can make dark circles look even darker.
This effect can be intensified by a flat sleeping position, salty food, alcohol, too little sleep, allergies, or a blocked nose. As the morning goes on, the swelling usually subsides with an upright posture, movement, and improved lymphatic drainage.
A cool compress, gentle movement, and a slightly elevated head position while sleeping can all help. Swelling that appears suddenly, is pronounced, or affects only one side should be examined by a doctor.
Why do some people have pronounced dark circles even at a young age?
Genetic predisposition plays a very significant role. Some people naturally have particularly thin lower eyelid skin, more visible blood vessels, a deeper tear trough, or a heightened tendency toward pigmentation. As a result, dark circles can appear as early as childhood or adolescence.
As we age, the skin loses collagen, elasticity, and density. At the same time, fat compartments and bony structures change. The mid-face loses volume, the tear trough can become more pronounced, and shadows grow more prominent.
Pigmentation disorders typically present as brownish discoloration. They can be genetically determined, intensified by UV exposure, or develop following chronic inflammation — for example in atopic dermatitis, allergies, or from frequent eye rubbing. In darker skin types in particular, post-inflammatory pigmentation is an important factor.
What can eye creams actually do about dark circles?
An eye cream can improve skin quality, but how much it helps depends on the cause of the dark circles. No cosmetic active ingredient addresses all types equally.
The best eye care is therefore not necessarily the most expensive — it is the one that addresses the underlying cause without irritating the sensitive area.
Caffeine serums and cooling eye patches are very popular on social media. Can you realistically expect visible results from them?
Yes, though expectations should remain realistic. Chilled eye patches and caffeine serums can visibly reduce morning puffiness and make the skin appear smoother through hydration. This can certainly be useful before an appointment or a photo shoot.
That said, they do not treat pigmentation disorders, volume loss, or a pronounced tear trough. A noticeable immediate effect should not be mistaken for lasting structural change.
People with sensitive skin should also steer clear of heavily fragranced patches or products with high concentrations of potentially irritating ingredients. The under-eye area in particular is quick to react with redness, dryness, or eczema.
Which home remedies actually help, and which ones are more myth than fact?
A chilled spoon or a cool compress can temporarily constrict blood vessels and reduce puffiness. Cucumber slices cool the skin and provide superficial moisture. The effect is primarily down to temperature and water content, however — not to any special anti-dark-circle substance in the cucumber itself.
Chilled tea bags can also have a decongestant effect. Caffeine and tannins could theoretically offer an additional benefit, but the classic tea bag method has not been convincingly demonstrated in clinical terms. Plant compounds or residues can also irritate the sensitive eye area.
I strongly advise against lemon juice, baking soda, undiluted essential oils, toothpaste, or aggressive homemade peels. These can cause irritation and inflammation, and may even make pigmentation worse.
Which aesthetic treatments are available as options?
Treatment must always be guided by the underlying cause. Hyaluronic acid fillers can create a more harmonious transition between the lower eyelid and cheek when there is a clearly defined tear trough and suitable tissue. For pronounced under-eye bags, excess skin, or a tendency toward lymphedema, they are often not the best solution.
Laser and light-based procedures may be considered for pigmentation, visible blood vessels, or declining skin quality. Pigment, vascular, and fractional lasers each target different concerns. Skin type, pigmentation tendency, and anatomy must all be carefully taken into account.
PRP, or platelet-rich plasma, is intended to support regenerative processes and improve skin quality. Individual studies and clinical experience point to possible improvements; however, the evidence is mixed and the effect is generally more subtle than social media portrayals suggest.
Autologous fat transfer can be a longer-lasting option for more significant volume loss, though the procedure is more complex and the healing response cannot be fully predicted. In cases of pronounced fat pads or significant excess skin, surgical lower eyelid correction may be more advisable than repeated injections.
When dark circles are driven by inflammation-related pigmentation, the first priority is treating the underlying allergies, eczema, or chronic irritation.
A considered combination of several measures is often more sensible than a single aggressive treatment.
Hyaluronic acid for dark circles has become an increasingly popular treatment. How safe is injecting the tear trough?
Tear trough injection is a demanding medical procedure — not a straightforward lifestyle treatment. With careful patient selection and highly experienced practitioners, it can deliver good results. However, it is not appropriate for every anatomy.
Possible side effects include swelling, bruising, asymmetry, nodules, overcorrection, and a bluish shimmer from the material known as the Tyndall effect. Hyaluronic acid can bind water and thereby worsen existing puffiness or under-eye bags. It can also persist in the tissue of this area considerably longer than many patients expect.
In very rare cases, filler can enter or compress a blood vessel. This can lead to circulatory disturbances, tissue damage, and in extreme cases visual disturbances up to and including vision loss. This is precisely why thorough anatomical knowledge, a very conservative dosage, and clear emergency protocols — including readily available hyaluronidase — are absolutely essential.
What matters is not simply whether a practice offers the treatment, but whether it also actively advises against it when individual circumstances call for that.
Are there treatments you would generally advise against?
Any standard treatment applied without first analysing the cause. Reflexively filling the tear trough with large amounts of filler when puffiness, pronounced fat pads, or excess skin are present can make the appearance worse.
Aggressive peels, improperly applied lasers, and high-energy home devices should also not be used indiscriminately on the thin skin of the lower eyelid.
With so-called skin boosters, polynucleotides, PRP, and other regenerative procedures, it is important to communicate transparently just how robust the evidence actually is. They may support skin quality in individual cases, but they are not a universal solution for every type of dark circle.
As a general principle: the closer a treatment is to the eye, the more important medical qualifications, a precise indication, and a conservative approach become.
If you could give just three evidence-based tips for dark circles, what would they be?
First: identify the cause. Pigmentation, blood vessels, fat prolapse, volume loss, and puffiness each require entirely different strategies.
Second: consistently protect the eye area and avoid irritating it. This means daily UV protection, sunglasses, a well-tolerated skincare routine, and treatment of any allergies or eczema. Frequent rubbing should be avoided at all costs.
Third: combine lifestyle adjustments and treatment realistically. Adequate sleep, low nicotine intake, moderate consumption of alcohol and salt, and a slightly elevated sleeping position can all help reduce puffiness. Structural or pigmentation-related dark circles, however, will not disappear entirely as a result. If they cause significant distress, a medically guided, cause-based treatment should be pursued.
