Eat for five days and still get the benefits of fasting? The fasting-mimicking diet promises everything from better metabolic health to slower aging. Here's what the latest research actually shows.
Eat for five days – and still fast? That is exactly what the so-called fasting-mimicking diet (FMD) promises. The method is designed to replicate the positive effects of multi-day fasting without requiring a complete abstinence from food.
The fasting-mimicking diet is currently considered one of the most exciting nutritional trends in the longevity world. Its proponents promise activation of cellular repair processes, improved metabolism, and even a slower rate of aging.
But how much of this is actually backed by science? And where do marketing claims outpace the current state of research? We have evaluated the most important myths in light of research available as of 2026.
The concept was developed by Italian aging researcher Prof. Valter Longo of the Longevity Institute at the University of Southern California. Unlike traditional fasting, food is not eliminated entirely. Instead, calorie intake is significantly reduced over five days: approximately 1,100 kilocalories on the first day, followed by around 800 kilocalories on each of the four subsequent days.
The diet consists predominantly of plant-based fats and carbohydrates, with deliberately low protein content. The goal is to trigger metabolic processes similar to those seen during multi-day fasting — even though small amounts of food continue to be consumed.
Is that true?
Partially. A study published in 2024 by Sebastian Brandhorst, Morgan Levine, and colleagues found measurable changes in liver and blood markers, suggesting a reduced biological age and lower disease risk.
However, such shifts in markers are not the same as the physiological processes that occur during multi-day water fasting — and, crucially, they do not constitute direct proof of increased life expectancy in humans. The available data comes predominantly from short-term observations; long-term data on hard endpoints such as mortality is still lacking.

Is that true?
The phrase "cellular rejuvenation," which is frequently used to market the diet, goes beyond what the studies actually show.
What has been documented are changes in various metabolic and inflammatory markers. In addition, early human studies suggest that the fasting-mimicking diet may stimulate autophagy — the body's own cellular recycling process. Whether this actually translates to a slowing of biological aging has not yet been scientifically demonstrated.
Is that true?
No. The longest and methodologically most rigorous study on the subject comes from primary care: a team led by Hanno Pijl of Leiden University Medical Centre followed people with type 2 diabetes over twelve months using a monthly five-day FMD program.
The result: on average, participants required less blood sugar-lowering medication while achieving better blood sugar control compared to standard care — though this was conducted under close medical supervision and with defined medication adjustments.
And that is precisely the point: whenever medications are involved, an unsupervised calorie reduction can become dangerous — for example, by causing hypoglycemia. The fasting-mimicking diet is also considered unsuitable, or only appropriate under medical supervision, for pregnant women, breastfeeding women, underweight individuals, and people with a history of eating disorders.
Is that true?
This cannot be determined at present. Studies exist for both the fasting-mimicking diet and various forms of intermittent fasting showing benefits for weight, blood sugar, and metabolism. However, direct comparison studies that clearly establish the superiority of one approach over the other are currently lacking.
A key difference lies in the concept itself: while intermittent fasting restricts daily eating windows or incorporates regular fasting phases, the fasting-mimicking diet is a time-limited five-day dietary intervention involving significant calorie reduction. This triggers partly different metabolic processes, which have not yet been sufficiently compared with one another.
Which approach is more beneficial therefore depends on individual goals and on which dietary strategy can realistically be maintained over the long term. For most people, a consistently balanced diet combined with regular physical activity is likely more important than the choice between the fasting-mimicking diet and intermittent fasting.
Is that true?
High-quality clinical data is now available on this question: a research team from Stanford University published a randomized controlled study in Nature Medicine in January 2026 examining FMD in mild to moderate Crohn's disease. After three months, 69.2 percent of participants showed significant improvement, and 64.6 percent achieved remission.
For a chronic inflammatory bowel disease that has had very few nutrition-based therapies with comparable evidence, this is a remarkable result. Important context: the diet represents a complementary measure, not a replacement for drug treatment, and the study applies to mild to moderate cases, not severe or complicated ones.
● Suitable: generally healthy adults looking for a time-limited, easy-to-plan dietary intervention
● Beneficial with medical supervision: people with type 2 diabetes or other metabolic conditions who are on medication
● Worth considering with medical supervision: mild to moderate chronic inflammatory bowel diseases such as Crohn's disease, as a supplement to existing therapy
● Not suitable: pregnant women, breastfeeding women, underweight individuals, and people with a history of eating disorders
● Not sufficiently proven: anti-aging and life extension claims in the strict sense
The fasting-mimicking diet is not a miracle cure — but it is not a purely marketing-driven trend either. The most convincing evidence so far relates not to anti-aging but to metabolic diseases, and now, for the first time, also to Crohn's disease. Those who look to the fasting-mimicking diet primarily for a longer life or "cellular rejuvenation" should, however, treat the existing results with caution.
