Falling asleep with earbuds and leaving them in until morning? ENT physician Dr. Kristina Wiedelmann-Breloer is seeing more patients with ear canal inflammation and eczema linked to their use. She explains what can happen inside the ear overnight — and how to reduce the risks.

Podcast, music, white noise or soothing nature sounds: many people put on headphones to fall asleep and end up wearing them all night long. Small in-ear headphones seem especially convenient. But wearing them for hours at a stretch isn't always as harmless as it might seem.
Dr. Kristina Wiedelmann-Breloer is now seeing the consequences regularly in her ENT practice: "The rise in earbud use has actually brought me a significant number of new patients suffering from ear canal inflammation, eczema, and allergic skin reactions around the ear."
The sounds coming from the headphones aren't the only factor at play.
"Because earbuds almost completely seal off the ear canal for extended periods, a warm, moist environment builds up inside — ideal conditions for bacteria and fungi," explains Wiedelmann-Breloer. "The problem is made worse by residual shampoo or shower gel in the ear, which can further irritate the already sensitive skin of the ear canal."
The headphones themselves can also irritate the sensitive skin. "The materials and coatings used in the headphones or ear tips shouldn't be underestimated," says the ENT physician. "In sensitive individuals, they can trigger irritative or allergic skin reactions."
The signs that ears aren't tolerating in-ears well can often be spotted early. "Common early warning signs include itching, redness, or flaky skin at the entrance to the ear canal. As the condition progresses, painful — sometimes even bloody — cracks in the skin can develop."
Earwax can also become a problem with prolonged wear. "Normally, the ear canal has a very effective self-cleaning mechanism," explains Wiedelmann-Breloer. "In-ears can disrupt this process and repeatedly push cerumen further inward."
This can have real consequences: "In the worst case, a complete blockage develops with associated hearing loss; at the same time, a cerumen buildup like this can promote inflammation."
Anyone who suddenly experiences reduced hearing, a feeling of pressure, or a blocked ear after prolonged in-ear use should not try to clean the ear themselves with cotton swabs. The WHO also advises against inserting any objects into the ear canal.
Beyond problems in the ear canal, prolonged exposure to sound overnight can also pose a risk.
"The far better-documented risk is the sound itself," says Wiedelmann-Breloer. "Volume and duration are the key factors. Prolonged or repeated exposure to high sound levels can permanently damage the delicate hair cells and other structures of the inner ear. Potential consequences include noise-induced hearing loss and tinnitus."
The WHO also uses a model that considers permissible listening duration and sound level together. As a safe listening guideline, it cites 80 decibels for a maximum of 40 hours per week for adults. For children, the reference value is 75 decibels for a maximum of 40 hours per week.
Anyone who only needs music or white noise to fall asleep should set a sleep timer rather than letting the headphones run until morning.
Wiedelmann-Breloer is not particularly worried on this front: "The current data is rather reassuring. So far, there is no confirmed evidence that the high-frequency electromagnetic fields emitted by Bluetooth in-ears damage the brain, auditory nerve, or inner ear. Exposure from Bluetooth headphones is also generally far lower than from a mobile phone, for example."
Other factors are therefore far more relevant for ear health: how long the headphones are worn, whether they seal off the ear canal, and how high the volume is.
Headphones designed specifically for nighttime use are now widely available. They typically feature very flat speakers housed in a soft headband, rather than sitting directly in the ear canal.
This offers at least one practical advantage over in-ears: the ear canal is not almost completely sealed off for hours by an ear tip. Whether sleep headphones are overall healthier for the ears, however, cannot yet be determined with scientific certainty. Reliable comparative studies focusing specifically on sleeping with different types of headphones are still lacking.
And even with sleep headphones, volume still matters. Those who only need sound to help them fall asleep can alternatively use a quiet speaker placed next to the bed — leaving the ear canal completely unobstructed.
No. "Earbuds are not inherently dangerous," says Wiedelmann-Breloer. "What matters is how loud, how long, and under what conditions they are worn."
The physician sums up the most important rules this way: "Taking regular breaks, keeping the volume moderate, and above all making sure your ears are clean and dry can already make a significant difference."
